• Full CV
• Passport style photo (digital picture image please, e.g. jpeg file, not a hard copy, pdf or copy of an image or copy of passport page) to make an identity badge
• Copy of latest DBS check certificate (if needed we can apply for a check for you, we pay half and you pay half and if this is the case we’d wait until work is clearly identified).  If you subscribe to the update service we still need to see a copy of the certificate to which that applies.
• Names, address, email and contact numbers for two clinical referees and your permission to approach them (they will not be contacted until you have confirmed that you are ready for us to do so).
• A copy of documentation showing your professional indemnity insurance cover period and amount (if you are an RCOT member a copy of your 2020/2021 membership card/confirmation of renewal e-mail will be all that’s needed – we cannot verify membership of a professional body online with only a membership number.
• Relevant education/training certificates (Diploma/Degree/Post graduate qualification and recent CPD)
• HCPC registration is confirmed online but a copy of your confirmation would be good.
